Summary of Job:

  • Collaborates with medical staff in the delivery of direct patient care as well as with appropriate members of other divisions to ensure optimal coordination of care for the patient
  • Participates in the development and implementation of individual, group and community education programs
  • Triage assessment process of walk-in patients and telephone calls
  • Delivers health education related to medication, diet, treatment of diagnosis, health promotion, prevention and exit health teaching
  • Administers immunizations, applicable contraception refills, and medications as ordered by provider.
  • Obtains and documents patient vital signs and other measurements, performs vision and hearing assessments, immunizations, applicable injections, and lab functions within scope
  • Ensures proper functioning equipment and prepares examination rooms daily ensuring that rooms are clean, orderly and properly stocked with supplies for use during exams.
  • Obtains health histories on patients, acts as a patient advocate, and provides full documentation of patient care/interactions in electronic medical record
  • Participates in quality assurance functions and interdisciplinary meetings
  • Assists in the coordination and delivery of health screening programs


  • Current Illinois RN or LPN licensure
  • Current BLS for Healthcare Provider CPR certification
  • Bilingual (Spanish/English) desirable for certain sites, may be required to utilize available translation services to communicate with non-English speaking patients and/or guests
  • Associate's Degree or Bachelor of Science in Nursing or the equivalent level of knowledge and experience
  • 1-2 years of experience in community health or ambulatory care preferred
