Goodwill TalentBridge in working with our client, a large health system with locations in Greater Milwaukee, is recruiting for a care navigator to work within a primary care team with pediatric patients and their families in areas of mental health/behavioral health support. 

In this role you would be working within our client’s pediatric health clinic as a part of a primary care team. 

ESSENTIAL JOB DUTIES

  • Coordinating patient and client services to provide timely and quality access to mental and behavioral health services 
  • Responsible for organizing and coordinating support services and treatment resources for the patients and their families.
  • Provide community resources and make referrals,
  • Check in with families regarding at home therapeutic interventions the therapist prescribed,
  • Assist with navigating school services such as the IEP process,
  • Complete safety check-ins with families between therapy sessions or while a therapist is out of the office,
  • Attend team meetings and support the families while in higher level of care services,
  • Advocate for the patient/family, assist with guardianships,  schedule follow-up services, navigate financial/insurance challenges, and remove barriers to receiving services.

 

Requirements:

Bachelor’s degree (or equivalent knowledge) in social work, psychology, public health, healthcare administration or human services required for this role.

 

SCHEDULE

  •   2nd shift with rotating weekends. 
  • Pay: $20 - $25/ hour
